How to Join

There are no appointments necessary to join Elemental Wellness. Stop by anytime during our business hours from 9am-9pm seven days a week to begin your free membership. You can also submit your membership agreement by using the Intake Form on our website. This allows us to contact your doctor and verify your patient status before you come in, saving you time, and ensuring you can enjoy all the privileges of membership on your first visit without delay.

When you arrive, we will ask you to present your original embossed doctor’s recommendation and a California picture ID. We will then give you a short but informative tour of the collective, including our clone nursery, holistic healing room, library, and medicine display cases. After that you’ll fill out our Membership Agreement form (if you didn’t submit it online) and then you’re ready to make your first contribution towards medicine. It’s that simple! Please note that due to San Jose regulations you must be at least 21 years of age to become a member, and are permitted only one visit per day. Before you can begin enjoying membership privileges we ask that you provide us with documentation.

What to Bring

1. Original embossed Doctor’s Recommendation (sorry, photocopies not accepted)

2. PLUS one of the forms of identification listed below:

      Current valid California DMV Driver License
      Current valid California Identification Card
      Expired California Driver License PLUS temporary valid California DMV Driver License
      Expired California Identification Card PLUS temporary valid California DMV Driver License
      Current valid out-of-state DMV Driver License PLUS temporary California DMV Driver License
      Current valid Passport PLUS temporary California DMV Driver License
Military Personnel

Please provide your original Doctor’s Recommendation plus your current United States Military Photo Identification Card PLUS out-of-state DMV Driver License with California address on it

 

Renewals

If you renew your Doctor's Recommendation, please bring your embossed Renewal (sorry, photocopies not accepted) plus one of the forms of identification listed above.

Primary Caregivers

Please provide your original embossed Doctor’s Recommendation (sorry, photocopies not accepted) PLUS one of the forms of identification listed above.

The primary caregiver’s qualifications and responsibilities include:

      A primary caregiver must be knowledgeable and attendant to the patient’s medical needs. A primary caregiver is not just a hired cultivator or supplier for the patient.
      A primary caregiver is required to keep records of a patient’s medical needs, including a copy of the physician’s recommendation. Ideally the caregiver will also be aiding in the journaling of a patient’s medical progress.
      A primary caregiver is required to keep records of the patient’s medical expenses.
      A primary caregiver MAY NOT grow/possess more than needed for the individual patient’s needs.
      A primary caregiver MAY NOT grow/possess more than above limits specified generally in the patient’s guidelines.
      A primary caregiver MAY NOT have more than three (3) qualified patients.
